What is Collaborative Documentation?

Collaborative documentation means writing session notes together with parents during the session. Instead of finishing paperwork afterward, the clinician and parent review and record the important details in real time. This ensures that what’s written accurately reflects the discussion, goals, and progress.

Why We Are Moving to Collaborative Notes

We are making this change to:

  • Improve clarity & transparency – Parents see exactly what is recorded and can confirm accuracy.

  • Save time & increase efficiency – Notes are completed during the session, freeing up clinician time outside of meetings.

  • Support engagement – Parents actively participate in treatment planning and progress tracking.

  • Enhance quality of care – Real-time notes strengthen shared understanding and keep everyone on the same page.

Benefits for Parents

  • Be more involved in your child’s care.

  • Ensure your voice is included in the notes.

  • Catch mistakes or clarify misunderstandings immediately.

Possible Drawbacks

  • It may feel different and take time to adjust.

  • Some families may prefer less focus on the computer or note-taking.

  • Sensitive topics may require flexibility in how notes are recorded.

If Collaborative Notes Don’t Work for You

Your comfort matters. If you feel collaborative note writing is not working for you:
